This website provides instructions and examples of how to assess students' understanding of both plant and animal processes. This website suggests that students must use their knowledge about characteristics and needs of plants and animals to create their ow animal, and it's habitat. This would be a great way to wrap-up the unit and assess students.

Although students at this age cannot write an extensive amount, have them write a short reflection on the unit. Have them answer the following question:
Animals (including humans) and plants require the same resources for basic survival. You have seen what happens to plants when they do not have access to these resources. Based on your observations, what would be the one resource that you think animals (including humans.. that means you!) could live without?

Students could complete a short research project for a final assessment of the unit.
For this project, students would have to complete the following:
1) Research (using classroom resources, library resources, personal experience, and any resources of your choice) to find an animal and a plant that live in the same habitat
2) Find two reasons that the plant lives in the habitat
3) Find two reasons that the animal lives in the habitat
4) Find one picture of the plant
5) Find one picture of the animal
6) Present the information that you found to a poster board, and show your findings to the class!
